If you never received it from T-Mobile in an email, or lost it, or can’t remember, just “Click here to request it.” I did this and within seconds I received an email with a unique class member id. You then use that to apply. This takes about two minutes. I chose to receive payment from Venmo because I don’t want to provide my banking information. You can also choose to pay with PayPal, a Mastercard gift card, or several other options.
