Intermountain Healthcare, Story Health’s Heart Failure Partner

Intermountain Health and Story Health, a virtual specialist healthcare company, announced a strategic partnership to enable healthcare patients with heart failure to receive care outside of the clinic.

California-based Story Health offers telemedicine, medical trainers, artificial intelligence and connected devices to treat patients with heart failure at home. It also allows doctors to access patient data and receive personalized recommendations based on the doctor’s prescription history.

As part of the partnership, Intermountain’s clinicians and medical technology company trainers will collaborate to provide patients with personalized treatment and monitor medication access and adherence between visits.

Coaches will act as intermediaries between patients and their providers, monitoring the patient’s progress and answering questions via text, email or phone.

“Patients go a long way with their health, and much of that journey is after they leave the doctor’s office,” Story Health CEO and co-founder Tom Stanis said in a statement. “Our solution combines the latest AI-integrated electronic health record technology with real medical trainers to directly identify and remove the most damaging barriers in the patient path without an internet connection. Story Health is excited to partner with Intermountain to transform the continuous patient care experience.”
